11/5/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Kaila upon arrival to the home. Upon arrival Kaila was combing her hair complaining that she was tired and overwhelmed. QMHPA engaged her in a discussion to determine why she was tired. She reported that she was only tired because the neighbors dogs seemed to want to be barking all night long and the apartments are so close so you can hear everything. Intervention: QMHPA began the session by assessing Ms. Mathew’s emotional well-being. QMHPA reinforced developing a list of chores she can complete that will help her decrease some of her clutter. QMHPA encouraged Kaila to listen to her music as a motivator she can use to help her with her chores. QMHPA modeled several coping methods Kaila can implement in her daily life to help reduce periods of depression. QMHPA encouraged and supported for using deep breathing exercises and positive self-talk to get her through her day-to-day responsibilities in her home. QMHPA focused on supporting and encouraging developing and practicing good decision-making skills based upon immediate need(s) and priority. This skill will help Kaila weigh her pros and cons in decision making. Response: Kaila appeared to be receptive of QMHPA’s suggestions that were provided to her during the session. Kaila was able to identify what her daily needs are for the next three days and schedule them accordingly. Kaila was supported for her willingness to stick with her plan to clean her living area and not make excuses. Kaila acknowledged that sometimes she finds it hard to make it through her morning and some nights because she deals with so much anger and aggression. Kaila was encouraged to begin thinking and talking positive. Kaila shared that she was tired of having all these bad days so she plans on utilizing the skill builders taught. Kaila was supported for making positive stride today and for future improvement. Kaila admitted that making some bad decisions and choices has created most of her depression, anxiety and feeling overwhelmed. Plan: QMHPA will educate Kaila on utilization of relaxation skills that consist of muscle relaxation, guided imagery, and slow diaphragmatic breathing. QMHPA will assist Kaila with coping and calming strategies to reduce depressive symptoms during individual sessions (1-2x weekly).
